On August 1, 2011, Attainment Company released its new Aging Resources book, Person-Centered Care in Practice: Tools for Transformation, by Lyn Geboy and Beth Meyer-Arnold. The book provides caregivers with a practical approach for implementing person-centered care, a promising mode for achieving culture change in elder and dementia care facilities.
Based on the authors’ research and real-world application in an elder and dementia care facility, the book provides caregivers with the foundation they need to make system-wide improvements. The action plan outlines nine basic strategies for implementing person-centered care, and a toolkit with two dozen operational tools helps caregivers assess progress. A CD with a printable PDF and a customizable Microsoft Word version of the Toolkit is included.
Person-Centered Care in Practice pioneers a movement of change in long-term care facilities for the elderly. “Changing culture is personal,” Beth says. “We must drive change for the people we serve, as passionately as if it were for ourselves.”
