The GUIDE Model will test whether a comprehensive package of care coordination and management, caregiver support and education, and respite services can improve quality of life for people with dementia and their caregivers while delaying avoidable long-term nursing home care and enabling more people to remain at home through end of life.

Jim Gera is CEO and Founder of Harmonic Health, a comprehensive dementia care clinic operating in 30+ states. As a caregiver for three family members with Alzheimer’s dementia he has brought his diverse experience in healthcare with physicians and payers across commercial, Medicare and Medicaid programs to improve care for patients and families living with dementia. He started and led two value-based care start-up companies which managed $4 billion in medical cost with over 10,000 physicians across 40 states serving over 95,000 patients annually. His companies have improved care and access to numerous patients and families and reduced adverse outcomes up to 40% and achieved satisfaction scores up to 98%.
William [Bill] Zagorski is the C.E.O. of American Senior Care Center, Inc. overseeing Centennial Adultcare Center’s three Adult Day Health Care locations, non-emergency medical transportation, and home care services, serving more than 150 adults each day throughout middle Tennessee. Bill is a Nashville native, who completed his undergraduate degree at Xavier University, and his graduate degrees in Cancer and Cell Biology, and Molecular Genetics, Biochemistry, and Microbiology at the University of Cincinnati, College of Medicine in Cincinnati Ohio. In addition to his role with American Senior Care Centers, Bill is also active on the Legislative Committee of the Council on Aging of Middle Tennessee, the past President of the Senior Services Network of the Mid-South, President of the Tennessee Association of Adult Day Services, the Chair of the Tennessee Coalition for Better Aging, and Chair of the Research Committee and Board Chair of the National Adult Day Services Association.

Falls are the primary cause of injury and fatal injury among older adults. More than one-third of people over the age of 65 fall each year resulting in 3 million admissions to the emergency department. Additionally, older adults who fall once are two to three times more likely to fall again. Even falls without injury can cause fear of falling, leading to physical decline, depression, and social isolation. This webinar will provide you with practical interventions to reduce falls and hospital readmissions.

Seth Walker is a passionate career professional in the I/DD space with a focus on program development, compliance, and participant driven results. Seth also has experience in Senior Services as a Director of Operations at an Adult Day Facility and as a Policy Consultant. A resident of Northeast Ohio, Seth has a Bachelor’s degree in Forensic Psychology and a Master’s in Social Work.
NADSA has revised its Standards & Guidelines manual which provides essential information to assist in starting up an Adult Day Services center. (Note that the information is designed to be applicable to most ADS centers and additional research on state and local regulations will be required.)
